Su carrera profesional empezó en Marvel en 1974, más concretamente como asitente de producción y colorista. En 1975 comenzó a escribir guiones para la colección The Deadly Hands of Kung Fu, donde permaneció hasta 1977.

Mantlo se convirtió en el rey de los fill-ins, o sea, las historias que se publicaban cuando los autores regulares de una serie entregban tarde. También fue el guionista de dos licencias de mucho éxito, The Micronauts, junto a Michael Golden, y ROM, con Sal Buscema, para las que colaboró entre 1979 y 1986.

Otras colecciones de Marvel que escribió fueron Alpha Flight, The Amazing Spider-Man, The Avengers, The Incredible Hulk, Iron Man, Peter Parker, The Spectacular Spider-Man, The Human Fly o The Champion.

También trabajó como guionista para Warren y DC Comics. En 1992 sufrió un grave accidente al ser arrollado por un camión, sufriendo daños irreparables en su cerebro. Una de sus creaciones, Mapache Cohete, sería uno de los principales personajes de la película Guardianes de la Galaxia, estrenada en 2014.
