Synopsis "Vaquez-Polyglobulie (in German)"

Die Vaquez-Polyglobulie (VP) ist ein myeloproliferatives Neoplasma, das durch die klonale Expansion einer pluripotenten hämatopoetischen Stammzelle entsteht und zu einer Proliferation des myeloischen Gewebes führt, bei der die erythrozytäre Linie vorherrscht. Sie ist durch eine Zunahme der gesamten Erythrozytenmasse und des Hämoglobins ohne übermä ige Stimulation durch Erythropoietin gekennzeichnet. Ihre Inzidenz ist gering (2-3 Fälle pro 100.000 Einwohner/Jahr). Die anfängliche Präsentation ist häufig pauci symptomatisch. Bei einigen Patienten führt das Auftreten eines thrombotischen Ereignisses oder von Symptomen, die mit dem Hyperviskositätssyndrom in Verbindung stehen, zur Diagnose PV.Die Entdeckung der JAK2-Mutation V617F hat die Diagnose von PV völlig verändert und ermöglicht nun nicht nur ein besseres Verständnis der pathophysiologischen Mechanismen, sondern auch die Entwicklung einer gezielten molekularen Therapie in Verbindung mit einer leistungsstarken molekularen Überwachung.
